
FMCSA (DOT) Exams

Thank you for trusting True North Health to conduct your DOT exam. Please review the following information carefully. Not doing so could delay your certification.
After scheduling your appointment, look for an email from CharmHealth to set up a Patient Portal account. This will allow you to register and complete the agreement forms in advance. This saves time and helps ensure we stay on time. It also allows you to get an appointment reminder sent to your email 1 day prior to your appointment.
When You Arrive
We are located at 1 Kennedy Drive in South Burlington. Park on the Dorset St side of the parking lot if able. The sidewalk leading to the office is in front of the handicap parking places by the blue postal mailbox. Go down the sidewalk, the office is the first door on the left (L1). Ring the doorbell, the door is locked.
You will be asked to give a urine sample upon arrival. Please do not use the restroom without obtaining a specimen cup. Urine is tested for glucose, protein, and blood. No urine drug testing is performed. Paperwork provided upon arrival will ask you about any substance use and medical conditions and history. Substance use, including use of marijuana is a disqualification for a FMCSA/DOT certification.
Payment of $150 is due at the time of appointment, regardless of determination. You will be provided with a receipt to provide your employer for reimbursement if necessary. If your employer would like to pay in advance they can contact True North Health to make arrangements to do so.
Please Bring:
Driver’s License (Must Not Be Expired)
Glasses if prescribed
Medication list, including dosages or all medication bottles. If you take any controlled substances such as (but not limited to) Lorazepam or Ambien (for anxiety or sleep), opioids (for pain) or stimulant medications (for ADD, ADHD, Binge Eating Disorder, etc) please respond to this email with the name of the medication. Some medications require further clearance and we can coordinate this prior to your exam. Not doing so could delay your certification and require you to return for a follow-up eval once completed. If you are unsure contact True North Health to ask.
Negative Sleep Apnea Test Summary (Only if you had a sleep test that came back negative since your last DOT exam). This will allow certification if you have risk factors that would otherwise require us to order a Sleep Study.
If you have any of the following conditions please bring the requested reports;
Obstructive Sleep Apnea
CPAP Compliance Report completed within the past 3 months (If you have sleep apnea).
If completed through UVMHN then no need to print, we can access the report. Simply remember to notify the examiner it was completed. Reports from elsewhere should be printed and brought with you.
Compliance report must be completed after the date of your last DOT physical. If you are coming for a 3 or 6 month follow-up exam ensure you get a compliance report completed after your last exam and prior to this upcoming exam. It doesn't matter if it's only been 3 months.
Diabetes:
If your PCP is not a UVMHN affiliate please bring a copy of your most recent Hemoglobin a1C.
If you use insulin to treat your diabetes, you must have maintained at least the preceding 3 months of ongoing blood glucose self-monitoring records measured with an electronic glucometer that stores all readings, records the date and time of readings, and from which data can be electronically downloaded.
Form MCSA-5870 must be filled out within 45 days prior to your DOT physical.
Heart Procedure or Tests:
Please notify True North Health if you have had any cardiac testing or procedure in the past via email, portal message, or phone call. Sometimes additional clearance from a cardiologist is needed.
If you have had any cardiac testing or procedure in the past, copies of such are needed, unless conducted with UVMHN or an affiliate in which case they can be accessed by True North Health.
